Jean Anne Johnson Scholarship Fund

Award

Friends and family of Jean Anne Johnson have contributed to this scholarship fund. There will be three individual scholarships awarded in the amount of $2,500 each; paid in two separate installments.

Qualifications

1. Students applying for the above scholarship must be graduating seniors of Caddo Mills High

School who will enter college after receipt of a high school diploma.

2. Students must include a copy of their high school transcript with submission of application.

3. Students shall have maintained an overall 80 academic average during their high school years.

4. Scholarship assistance offered for the first year of post-secondary education shall be in two separate installments, the second dependent upon the student’s progress as follows:

· An academic average of 2.5 on a 4.0 scale shall be maintained during the first semester in order for a student to qualify for the remainder of funds to be applied during their first year.

5. Enrollment of students shall be “full-time” both semesters as determined by the full-time enrollment policy of their particular school.

Non-Renewable Provision

There is no provision for renewal of scholarship assistance. Funds are available on a one-time basis to students engaged in their first year of post-secondary education.

Payment of Scholarship Funds

· Two payments each will be awarded annually to each of the recipients.

· All scholarship and grant monies shall be channeled directly to the recipient’s college by arrangement made prior to matriculation.

· All monies awarded must be redeemed within one calendar year of the award date or will remain in the fund.

Recommendations

As part of the application procedure, student shall arrange for three personal recommendations from a non-family member. Example: pastor, teacher, school faculty member or other adult outside the applicant’s family.

Selection Process

Selection of the recipients shall be made by the Scholarship Fund Committee based upon such factors as need, academic achievement, citizenship, extra-curricular activities, etc. In addition, applicants will be requested to appear before the selection committee for a personal interview.

Instructions

1. Applicants shall complete Parts I, II, and III of this application. Upon completion, the applicant shall return these parts to the counselor who will forward them to the scholarship selection committee.

2. Applicant must submit three recommendations. Applicant shall provide the counselor or the principal with Part IV and two different teachers or other recommenders with Part V and Part VI. These parts are to be completed by the recommenders. The applicant shall turn in the recommendations enclosed in envelopes that have been sealed by the recommenders along with the completed scholarship application. The counselor will forward the recommendations directly to the scholarship selection committee. The applicant should ensure that his/her name is on the line provided on Part IV, Part V and Part VI and on the sealed envelopes.
